Pathways for Healthy Transitions

The Village Network of Cleveland has been working in partnership with the Cuyahoga County Department of Children & Family Services (DCFS) for over a year now to serve as Care Coordinators.

Our trained professionals work with and assist DCFS employees by identifying resources for high risk and difficult to place youth that have been in residential care for an average of 57 months. The goal is to return the children to their communities with formal and informal wrap around supports for successful transitions.

Originally 12 youth were involved with the program. Seven of the original 12 were successfully transitioned to residing with a family member, foster parent or independent living. Currently there are 16 children in the program.

"The National Wrap Around Initiative is changing the way human services does business," said Jennifer LaCortiglia, program director of The Village Network of Cleveland. "We are working together with Cuyahoga County to build better transition paths for many of the high risk and difficult to place youth."
